Masseter Botulinum Toxin Applications

3/10/2026 Authors: Dr. Levent Ünal

Masseter botulinum toxin treatment is one of the advanced toxin procedures in medical aesthetics that provides both aesthetic and functional benefits while requiring careful planning and assessment. Masseter muscle hypertrophy may contribute to increased lower face width, a more masculine facial appearance, and functional complaints such as bruxism. For this reason, treatment should not be approached solely as a facial slimming procedure but rather through a comprehensive evaluation of the entire masticatory muscle complex.

The primary objective of masseter botulinum toxin treatment is not to completely deactivate the muscle, but to modulate excessive muscular activity in a controlled manner. Before treatment, factors such as muscle volume, mandibular structure, history of teeth grinding or clenching, and facial symmetry should be evaluated together. Individualized treatment planning, rather than relying on standard dosing protocols, is essential for preserving functional balance while achieving natural aesthetic outcomes. Excessive dosing may result in reduced chewing strength, imbalance in lower facial contour, and decreased patient satisfaction.

One of the anatomical structures frequently overlooked during masseter botulinum toxin planning, yet highly influential on clinical outcomes, is the temporalis muscle. Working in conjunction with the masseter, the temporalis plays a significant role in mastication and may demonstrate marked hyperactivity, particularly in patients with bruxism. In some cases, injections limited to the masseter muscle alone may shift the chewing load toward the temporalis muscle, preventing complete symptom control. Therefore, in advanced or treatment-resistant cases, temporalis muscle injections should be considered as a complementary approach to masseter botulinum toxin treatment. The goal is to maintain physiological balance among the muscles of mastication while providing both functional relief and aesthetic harmony.

Throughout the training program, participants gain detailed knowledge of masseter and temporalis muscle anatomy, palpation techniques, safe injection zones, and the principles of muscular balance. When properly planned and, when indicated, supported by temporalis muscle injections, masseter botulinum toxin treatment can deliver highly satisfactory functional and aesthetic outcomes without compromising facial harmony.
