Dermal Filler Applications – Marionette Line Correction

Marionette lines are among the most common aesthetic concerns that contribute to a downward facial expression. Extending from the corners of the mouth toward the jawline, these lines can create a sad, tired, and aged appearance. When properly planned, marionette filler treatments can provide a significant improvement in facial expression and overall facial rejuvenation.

The development of marionette lines is influenced by several factors, including lower-face volume loss, diminished chin support, and the effects of gravity. However, volume depletion is not the only contributing factor. The dominant activity of depressor muscle groups—particularly the depressor anguli oris (DAO) muscle—also plays an important role in pulling the corners of the mouth downward. For this reason, treatment of the marionette region should not be approached solely as a local volumization procedure. Instead, it should be planned through a comprehensive assessment of lower-face balance and muscular dynamics.

This is where the concept of myomodulation becomes particularly important. Dermal fillers can be strategically positioned not only to restore volume but also to support or influence muscular activity. When placed appropriately, filler treatments can create a more controlled and natural effect on facial expression. Supportive filler placement within the correct anatomical plane may reduce the influence of muscles responsible for downward pull at the corners of the mouth, thereby providing subtle upward support. When indicated, combination treatment with botulinum toxin can further optimize the balance between muscular activity and structural support, resulting in more sustainable and harmonious outcomes.

Within the Marionette Filler Training program, participants gain detailed knowledge of regional anatomy, the interaction between muscles and fillers, principles of myomodulation, and safe injection strategies. The course teaches practitioners how to analyze the aesthetic relationship between the marionette region, chin, jawline, and perioral area. Potential complications—including overcorrection, facial heaviness, asymmetry, and alterations in mouth-corner movement—are also examined in depth.

The training philosophy extends beyond simply filling marionette lines. Instead, it focuses on creating a dynamic balance that elevates and refreshes facial expression. The objective is to develop the clinical awareness required to achieve a more rested, positive, and natural appearance without creating the impression of excessive volume enhancement.
